Why Blue Swan exists

Genuine medical care at home, built by a son who has lived this

John Cham spent over 15 years running mobile allied health services across Perth and Melbourne. In those same years he watched his own mother slowly burn out caring for his grandmother, who was living with dementia and bedbound after a stroke. He came home to Kota Kinabalu to build the kind of service he could trust with his own parents.

Two ways we care

One of two care plans, matched to your parent

Care is guided by how complex the situation is. Some situations are safely nurse-led; others need a doctor to assess, prescribe and lead.

Nurse-led transition careNurse-led

Transition Care Plan

Just discharged from hospital? For patients recovering from surgery, a wound, a feeding tube, a fall, weakness, confusion, or a change in medication, a nurse-led plan can start within days.

  • Nursing assessment & monitoring
  • Medication checks & wound / post-op care
  • Tube-feeding support & warning-sign guidance

Managing a condition that keeps changing, several health issues at once, or a frailty that's harder to handle at home? Our doctor-led plan brings the decisions to your parent's bedside.

  • Doctor assessment, prescribing & treatment planning

Common questions

What families ask us first

QIs Blue Swan a nursing home or a caregiver agency?

Neither. We do not take your parent anywhere, and we are not a maid or companion agency. We bring doctors, nurses and physiotherapists into your parent's own home, working together as one coordinated team.

QCan a doctor visit my parent at home in Kota Kinabalu?

Yes. Visits are doctor-led or nurse-led depending on how complex the situation is, and every plan is built on our clinical framework, and when a case needs a specialist decision, it goes to the right specialist, whether him or your own. A specialist home visit is arranged in select cases based on clinical need.

QWho actually provides the care?

A team of senior nurses and doctors, founded by John Cham with co-founder Dr Gordon Pang. When a case needs a specialist decision, it goes to the right specialist, whether him or your own.

QHow much does it cost?

Pricing depends on your parent's needs and is shared during your complimentary care planning discussion, so you get a clear, tailored answer rather than a generic price. We explain fees clearly before any care begins, and there is no obligation after that first discussion.

QHow do we start?

You start with a complimentary, no-obligation care planning discussion, by phone, video call, or at our office in KK Specialist Centre. We use it to understand the situation and recommend a suitable plan.
